Our Hospice Philosophy

Caring for someone at the end of their life can be a privilege, a blessing and an honor.  This is a selfless time, and caregiving is a gift that rewards both the patient and the family. We are here to make a difference; to relieve suffering, maintain dignity, and comfort people living with a life-limiting disease.


Hospice is not a place, it is a commitment to caring for those who have a terminal illness.  Personalized care can be provided in the comfort of your own home, an assisted living facility, or a skilled nursing facility. 


Hospice care neither prolongs life or hastens death. The goal of hospice care is to improve the quality of a patient’s last weeks, days and hours by offering comfort and dignity.

Meet The Team

Hospice Medical Director

 

Our hospice Medical Director, an expert in pain and symptom management, works in conjunction with your attending physician and our Elite Hospice team to manage and support your plan of care.

Your Attending Physician

 

The plan of care will be overseen by the patient’s current attending physician. The Elite Hospice RN Case Manager will work with this physician to follow any medication changes, treatments and other related medical information as indicated in the plan of care. Our Medical Director can also step in this role.

Hospice Nurse

 

Our hospice nurses are experienced in assessing and monitoring pain, symptom management as well as end-of-life issues regarding your terminal diagnosis. The hospice nurse will coordinate care with the hospice team as well as physicians to determine the most appropriate plan of care. The hospice nurse will make regular visits to ensure the patient’s comfort and needs are being met.

Home Health Aide/CNA

 

Provides in-home care to individuals who need assistance with daily living activities. They help with personal cares, such as bathing, dressing, and grooming, as well as with tasks like light housekeeping, and medication reminders. They also offer companionship and emotional support, helping patients maintain their quality of life wherever they call home.

Social Worker

 

Hospice Social Workers are available to provide counseling and emotional support as related to the patient’s diagnosis. Social Workers can also help utilize community resources for financial assistance and funeral arrangements. Our Social Worker will also support the patient’s family for 13 months after the loved one’s passing. Mailings, support groups and grief counseling are just some of the offerings.

Volunteers

 

Our Volunteers are trained individuals to provide companionship. This may include but not limited to: reading to the patient, playing card games, pet therapy or just spending time conversing.

Chaplain

 

Hospice Chaplains provide non-denominational spiritual support while in our care. Chaplains can also work with the patient’s local clergy to create a broader scope of spiritual care for the patient and family.

Other Services

 

At Elite Hospice we also understand the need for other services that can include Physical Therapy, Occupational Therapy and Speech Therapy.

Life Enhancement Program

 

This program is designed to individualize patient care holistically based on lifelong preferences and interests. Techniques include but are not limited to;

music enrichment, aromatherapy, and video conferencing to keep in contact with family members.

Video Conferencing

 

Through various software applications, users are able to make voice calls, message and video conference over the Internet. Patients and families will be able to see each other and have “face-to-face” conversations
